I have an 11 yr old boy who has had this problem as long as we can remember. He says its mildly painful. Sometimes it's one ear, sometimes both...hot to the touch and red, red, red. He has ADHD and significant hay fever. I have always assumed this is allergy related, though he takes claritin (and before the zyrtec) daily. The claritin helps the runny nose/sneezing/itchy eyes, but doesn't eliminate the ear thing.
I'm fairly convinced this a food sensitivity or allergy of some kind.
